Family Case Management

Family Case Management is a case management program whose

goal is to promote wellness and healthy lifestyles in income-eligible women, infants, and children. The case management program can help you:


- Find a doctor for prenatal care and family planning

- Find a doctor for your child

- Learn parenting skills necessary to raise a happy, healthy baby

- Assess and provide information about your infant or child's development

- Obtain community programs and services

- Assist in application for a temporary medical card for pregnant women

- Make sure that your home is safe for an infant or child.
